Transparency in Health Care

The Florida Agency for Health Care Administration (AHCA) serves as the chief health policy and planning entity in Florida. The agency is responsible for administering Medicaid, licensing and regulating health facilities and providing information to residents about the quality of health care they receive in the state. The website below contains performance and outcome data for health care facilities in Florida as well as information on selected medical conditions and procedures. If you would like an estimate of charges for services provided, please contact the Chief Executive Officer (CEO) of the hospital at 352-554-2200.

The estimate for services (service bundle) is a non-personalized estimate of costs that may be incurred by the patient for anticipated services and that the actual costs will be based on services actually provided to the patient. Note that some services may be provided in our hospital by other health care providers who may separately bill the patient and that provider may or may not participate with the same health insurers or health maintenance organizations as our hospital. Additionally, the hospital has contracted for certain services with UF Health.

If you see your insurance provider listed below, it is still a good idea to check your individual policy or contact your insurance company to ensure your plan covers treatment. Physician Services

UF Health Rehab Hospital contracts with health care practitioners and medical practice groups to provide physician services in our facility. This physician list is the physicians we have current contracts with and these physicians may bill separately for their services. You should contact the practitioner or group who will be providing your services to determine the health insurers and health maintenance organization with which they participate.
